Sciatica and herniated disc treatment in Limassol

Pain that shoots from your lower back down the leg, a foot that tingles or goes numb, sitting that has become hard work, and an MRI that says “herniated disc”? Dr Zano Tyrannis first finds out where the pain is really coming from, then treats it with chiropractic care, needling and exercise. A non-surgical approach, with an honest answer if you need a doctor instead.

Back pain with leg pain: the signs people come to us with

01

Pain that runs down the leg

It starts in the lower back or buttock and travels down the back or side of the thigh, sometimes below the knee into the foot. Often the leg hurts more than the back.

02

Numbness or tingling

Pins and needles in the leg, foot or toes, or a leg that feels weak or heavy.

03

Worse when you sit or bend

Long drives, a day at the desk, putting on socks. Coughing or sneezing can send a jolt down the leg.

04

An MRI that worries you

The report says “herniated disc”, “protrusion” or “bulge” and you are wondering if that means surgery. Bring the scans with you.

05

It keeps coming back

Better for a few weeks, then the same pain again after lifting, gardening or a long flight.

06

Not sure it is sciatica?

Leg pain can also come from the sacroiliac joint, the piriformis muscle, the small facet joints of the spine or the hip. They feel similar and need different treatment, which is why we examine before we treat.

What sciatica and a herniated disc really are

Based on Chapter 4 of The Back Pain Blueprint, Dr Zano’s book on back pain, “Disc Pain: The Most Misunderstood Diagnosis”.

Diagram: a healthy intervertebral disc and a herniated disc pressing on a nerve root
Diagram from The Back Pain Blueprint: a healthy disc (left) and a herniated disc pressing on a nerve root (right).
  • The disc does not “slip”

Each disc is firmly attached to the vertebrae above and below it and cannot move out of place. What happens is that the soft, gel-like centre pushes outwards through a weak spot or tear in the tough outer ring. That is a herniation. It can hurt a lot, but it is not the broken, unstable spine that the words “slipped disc” suggest.

    • Bulge. The disc widens evenly all round. Very common, often painless.
    • Protrusion. The centre pushes into the outer ring but not through it.
    • Extrusion. Disc material has pushed through the ring.
  • Sciatica is a pattern of symptoms

The sciatic nerve is the largest nerve in the body. It is formed from nerve roots in the lower spine and runs down the back of the leg to the foot. When one of those roots is pressed or irritated, most often by a herniated disc, you can feel pain, tingling, numbness or weakness anywhere along its path.

True sciatica follows a defined line, often comes with numbness or tingling, and can be reproduced by specific tests such as the straight leg raise. That is why it should be diagnosed by examination, not by where the pain travels.

  • Your MRI is not the whole story

Studies of people with no back pain at all have found bulges, herniations and worn discs on many of their scans. So a disc change on your report may or may not be the cause of your pain, and pain can come from structures that look normal on MRI. We treat you, and use the scan as one piece of the picture.

  • Reason for optimism

Repeated MRI studies show that most lumbar disc herniations shrink over time, and many disappear completely. Larger ones are often the most likely to shrink, because the body treats the escaped disc material as foreign and clears it away.

It takes weeks to months, and it is rarely a straight line. Leg pain usually eases first, then numbness and tingling, and the local back pain last. Conservative care is the first step for most people, and surgery is kept for specific situations.

“MRI scans the structure of the spine. It cannot scan the pain.”

Safety first

When you need a doctor first, not a chiropractor

Most back and leg pain is mechanical: painful, but not dangerous. These signs are the exception. If you have any of them, do not book with us first.

  • Loss of bladder or bowel control, or new difficulty passing urine
  • Numbness in the “saddle” area: groin, inner thighs, genitals or buttocks
  • Leg weakness that is getting worse, in one leg or both
  • Fever or chills with back pain, or feeling generally unwell
  • Unexplained weight loss, or a history of cancer with new back pain
  • Back pain after a fall, a car accident or another serious injury
The first three can be signs of cauda equina syndrome, a surgical emergency. Go straight to the hospital emergency department or call 112. Do not wait for an appointment, including one with us.
We check for these signs at every first visit. If chiropractic care is not right for you, Dr Zano will tell you and suggest who to see, and you do not pay for that visit.
How we treat

How we treat sciatica and disc problems

No two backs are the same, so the plan is built around your examination, not around the scan.

  1. Examination first

Dr Zano asks how the pain started and what makes it better or worse, looks at any MRI or X-ray you bring, and checks movement, strength and sensation in your legs. The aim is to find where the pain is really coming from.

  2. Chiropractic adjustments

Precise, hands-on adjustments of the spine and joints, or slower, gentler mobilisation when that suits you better. The technique depends on what the examination shows and on how acute the pain is.

  3. Acupuncture or dry needling if indicated

Fine needles for tight muscles and trigger points that add to the pain, alongside chiropractic care.

  4. Exercises and everyday advice

Specific exercises to support your spine as it recovers, and practical advice on sitting, lifting and sleeping between visits.

Sciatica & disc programme

12 weeks, as many visits as you need, one price

Disc problems and sciatica usually need regular care over several weeks, not one visit. So we made the 12-week programme: you come as often as your plan needs, and pay once.

€720for 12 weeks,
unlimited visits
  • Unlimited visits within 12 weeks
  • Chiropractic care, plus acupuncture or dry needling if indicated
  • €720 is the price of 12 follow-up visits, everything above that is included
  • Whether the programme suits you is decided together at the first visit

Why regular care matters. Pain often eases before healing is finished: the inflammation settles first, while the joints, the deep supporting muscles and the disc itself take longer. People who stop as soon as it stops hurting are more likely to have the pain come back. Twelve weeks gives recovery time to happen.

Questions

Sciatica and disc questions

Anything else, just ask. We answer on WhatsApp and Telegram.

Ask on WhatsApp

Do I need an MRI before my first visit?

No. Sciatica and disc problems are diagnosed mainly by examination, and many disc changes on MRI cause no pain at all. If you already have an MRI or X-ray, bring it. If Dr Zano decides you need an X-ray before treatment, he will tell you, and you don’t pay for that first visit.

Can a chiropractor help with a herniated disc?

For most people with a herniated disc, care without surgery is the first step: hands-on treatment, exercise and advice while the disc settles. Whether chiropractic care suits your case is decided at the first visit, after the examination. If it doesn’t, Dr Zano will tell you and suggest who to see instead.

Will I need surgery?

Most people with a herniated disc do not. Surgery is the right choice in specific situations: cauda equina syndrome (loss of bladder or bowel control, saddle numbness), leg weakness that keeps getting worse, or severe leg pain that has not improved after a proper course of conservative care, usually at least 6 to 12 weeks. If your symptoms point that way, we will refer you to a doctor.

How many visits will I need?

It depends on the problem and how long you have had it. Disc problems usually need regular care over several weeks: leg pain tends to ease first, then numbness and tingling, and the back pain last. For this we have a 12-week programme with unlimited visits for €720. After the first examination you will know what Dr Zano recommends.

Does treatment hurt?

Adjustments are usually not painful. Some people feel mild soreness for a day or two afterwards, much like after exercise. When the pain is acute, Dr Zano can use gentler, slower techniques, and he explains each step before he starts.

Which languages do you speak?

Consultations are in English. Dr Zano also does a basic consultation in Russian, and replies in Russian on WhatsApp and Telegram.
